Referring a participant to Loving Hearts

If you’re a support coordinator, this page covers what to check before you refer: our registration, the supports we deliver, the areas we cover and how referrals work. Loving Hearts Care Group is a registered NDIS provider based in Helensvale. Refer online, call 0415 294 752 or email info@lovinghearts.com.au.

What to check before you refer: our registration, supports, areas and referral steps. Refer online or call 0415 294 752.

Our registration

The supports we're registered to deliver

Our registration is listed as Approved on the NDIS Provider Register and is in force until 27 March 2029. You can check our registration on the NDIS Provider Register. We hold these 12 registration groups, named as the register names them:

Each linked group opens its service page, and our NDIS services page lists the supports we provide.

If a participant is new to this, our guide to what NDIS registration means explains it in plain English.

We're registered for everyday supports, not plan management, therapy or behaviour support.

Areas we cover

Where we support participants

We support participants in 14 Gold Coast suburbs: Helensvale, Coomera, Upper Coomera, Pimpama, Nerang, Southport, Oxenford, Pacific Pines, Hope Island, Coombabah, Runaway Bay, Labrador, Parkwood and Ormeau.

If your participant lives just outside these suburbs, call 0415 294 752 to check, or see the areas we support.

How referral works

From referral to first contact

Referring takes a few minutes. Here’s what happens next.

Send the referral

Use our online form, or call or email us. The form asks you to confirm you have consent.

We get in touch

We respond during business hours, and contact the person you name on the form.

We plan the support together

We talk through goals, routines and preferences, then match a support worker.

Before you refer

What to include in a referral

The form asks for your details and role, the participant’s name and suburb, who we should contact, and the support needed, with preferred days, times and a start date. It also helps to tell us:

Prefer to talk it through?

FAQs

Questions from support coordinators

Do I need the participant's consent before referring?

Yes. Our referral form asks you to confirm you have the participant’s consent, or appropriate authority, before you share their details. The NDIA also shares plan information with providers only when the participant agrees.

Yes. We’re a registered NDIS provider, and participants with NDIA-managed funding must use registered providers. Plan-managed and self-managed participants can use us too.

Our registration is in force until 27 March 2029, and the register lists the groups it covers.

Yes. On the referral form you choose who we contact first: the participant, a family member or carer, a guardian or nominee, the support coordinator, or you as the referrer.

The form also asks for the support needed and the preferred days and times, so the first conversation can cover them.

The notice period is whatever their current service agreement says, because there’s no universal NDIS notice period.

Our guide to changing NDIS providers covers service agreements, notice and how to avoid a gap in support.

It depends on the participant’s needs, so call us on 0415 294 752 to talk them through.

Short-term accommodation is claimed under the Daily Tasks/Shared Living registration group, which we hold. Our page on NDIS respite explains how respite can work.
